Applicants are required to submit the following documents together with the Business Name Registration Application Form.

1. Recommendation Report of the Grama Niladhari of the Relevant Division
2. Development Coordinator's Recommendation
3. ecommendation Report from the Regional Director of Health Services
4. Environmental Officer's Recommendation Report
5. rade License Issued by the Pradeshiya Sabha or Recommendation from the Development Officer of the Relevant Division
6. Proof of Ownership of the Land on which the Business is Operated
7. Authorization Letter from the Owner
8. If the property is mortgaged to a bank, a certified copy issued by the Bank Manager must be submitted.
9. If the property is under a lease agreement, both the original lease agreement and a photocopy must be submitted.
10. Certified copy of the National Identity Card (NIC) must be submitted.
11. Evidence of registration under the Social Security Pension Scheme must be provided.
